Amendment 3 dated 1/3/12 has been uploaded and is ready for you to download and submit to your IRBs. You can keep accruing under the old protocol until the amendment is approved by your IRB.
The changes incorporated in Amendment 3 and Amendment 2 include the new
“Human Biological Sample Questionnaire”. In order to make downloading and using
forms more user friendly we also made a change to all the forms. Next to the
title of the form you will now find the corresponding amendment version number,
in this case you will see “Amd3” as
the version on all the forms you need to use for this amendment whether the
forms were modified or not. This has been done to all the forms so that there
will be no question as to what version of the form you are using. As soon as
your IRB approves the amendment all the forms you will be using will be labeled
“Amd3”, this is also valid on our website. Please refer to the “summary of
changes” for amd2 and amd3 to see the forms for which content has been updated.
Please download the "10055 Amendment 3 Complete PDF" and the new consent form to submit to your IRBs for approval. |

Now that accrual has ended for
the curcumin study, we are getting questions about what to do with all the
supplies and equipment required for that study. Once all of your CCOP’s
participants have completed the study requirements, follow the table below to determine
the correct action for each item.

URCC CCOP Research base 2012 annual meeting |

SAVE THE DATE!
The URCC CCOP Research Base annual meeting has been scheduled for Sept 6-8, 2012. It will be held at the Rochester Hyatt Riverside, in downtown Rochester. The format of the meeting will be the same as last year: coordinators’ session Thursday night (investigators are welcome to attend also), general session is all day Friday and Saturday morning.
More information will follow as the meeting time draws closer. |

For the Cognitive
100055 study:
•
Control participants DO NOT complete the FACT-G (only subjects receiving
chemotherapy).
For all
studies:
•
If you do not have a -80 freezer, please place “X” in those boxes on the blood
requisition form.
•
Please use black pen on all forms except for the trail making paper tests
(please use pencil for those forms).
•
No staples or hole punches in the forms. Paperclips are fine. |
